Care Coordination
The Palmer Senior Citizens Center's qualified, experienced care coordinators are trained to identify physically and emotionally frail seniors and help them to maintain independent, healthy, and dignified lifestyles. Our care coordinators will assess a senior's unique circumstances and, working with the individual and the individual's family and caregivers, develop a detailed plan of care. Our care coordinators provide a variety of services, including:
- Outreach
- Education
- Advocacy
- Referral
Who qualifies? Individuals eligible to receive care coordination services from the Palmer Senior Citizens Center include:
- Individuals age 60+
- Adults of any age with Alzheimer’s Disease or Related Disorders (ADRD)
- Adults with physical disabilities
Living Well Care Coordination
The Palmer Senior Citizens Center manages a specialized program, the Living Well Care Coordination program, that provides outreach, education, advocacy, and referral services for seniors/disabled adults with mental illness and/or substance abuse concerns. The Palmer Senior Citizens Center is certified by the State of Alaska to provide care coordination services under the Medicaid Waiver (CHOICES) program. |
